All you need to know is what you can get, not where it is in the menu. You can figure the menu out when you make your wish.
You don't press a "I want the transformation skill" button and that's it. You press that button, it will show you an icon of the SSJ transformation, and then you either press "yes" or "no".
You apparently can't make any wishes for the level 50 items/skills until you're level 50. So you can only get either the stat buff, the level 40 HTB skill (assuming you're level 40+), or the Zeni. For the stat buff, you can tell which buff you're getting because the name of the buff ends in the name of the stat it buffs. For example: 용신의
힘 is the buff that increases strength.
In the order they appear on the character sheet:
힘 - Strength
민첩 - Dexterity
근성 - Constitution
기운 - Energy
기합 - Soul
집중 - Focus
100
